PASSPORTS

This office is the acceptance facility for passports for Mineral County. We have the applications in our office or you can find them online at www.travel.state.gov . The cost of a passport is now $135 and is good for 10 years. The processing centers are running behind with the volume of applications and it is presently taking up to 4-6 weeks to receive your passport. If you are traveling within that time frame, they offer an expedited service which costs an extra $60 plus overnight delivery both directions.

Applications for renewal by mail are also available here or on the website: travel.state.gov

To apply for your passport you will need an original or certified copy of your original birth certificate; two identical passport photos; and a picture ID. Your photos can be bought at Costco for $4.99, (you do not need to be a member) or Walgreens for $7.99, or Kinkos for $12.95.

MARRIAGE LICENSE

To obtain a license, the female applicant will need a pre-marital blood test for rubella (German Measles) performed by a doctor, clinic, or hospital lab. After the Doctor has signed the “pre-marital certificate” the couple should apply for the license and present the certificate. If she is under 50 years of age and incapable of bearing children, she must present a certificate of exemption, signed by a doctor when applying for the license. She does not need a blood test or certificate of exemption if she is over the age of 50. As of October 2007, an Informed/Consent/Waiver of the Requirement of Blood Test is available.

To obtain a copy of the Pre-Marital Certificate/Informed Consent and Waiver, please visit the Health and Human Services web site.

The application for marriage is very self-explanatory. Please fill out the fields 5a through 15d as well as 35 a,b,c. Do not sign the application until asked to do so by the Clerk of Court's Office. A copy of the application can be found here.
